Athletes and Families,

All athletes may get their sports physicals at the Lamphere Health Center. They must have a scheduled appointment, a consent form, and the MHSAA sports form completed prior to the physical (you may get these from the Main Office). They can call 248-849-4823 to schedule their appointment, or they can walk into the health center to schedule and pick up the forms needed. Someone will be here Monday through Friday to answer the phone but physicals will only be performed on Wednesdays. Our services are no cost to families, but spots are limited.
