Before any student athlete can participate in interscholastic athletics at Page Middle School, they must take care of the following responsibilities:

1. Completed Sports Physical, dated after April 15th and signed by a licensed physician.
   You might be able to get a FREE Physical at MedPro Primary and Urgent Care. Call them at 248-850-8266.

2. Completed Big Teams Profile- under the STUDENT'S NAME
   1. Health History Form completed and signed by athlete and parent
   2. Consent and Insurance Form signed by athlete and parent
   3. Concussion Form completed and signed by athlete and parent 

1. Directions for completing Big Teams Profile
2. Have all other (previous) sports equipment successfully returned
3. Please complete the Student Athlete Code of Conduct Form. You will only need to do this once
